AIEI Membership Questionnaire
Welcome to the AI Ethics & Integrity International Association (AIEI) Membership Questionnaire. This comprehensive survey is designed to gather essential information about your organization’s AI ethics maturity, governance, and practices. It is intended for organizations across all sectors – that develop, implement, or use AI products and services. Your candid and thoughtful responses will help both your organization and AIEI understand the current state of AI ethics readiness and identify areas for growth.
Confidentiality: All information you provide will remain confidential within AIEI. We encourage honesty and self-reflection. You do NOT need to disclose any proprietary or sensitive data. When giving examples, you can describe scenarios in general terms without naming specific products or secrets.
⚠️ Important! The information you provide in this questionnaire will be used for research purposes to help AIEI improve the support, resources, and benchmarking insights we offer to our members.
Please note: As governments and policy makers often face challenges keeping pace with the rapidly evolving AI industry, we may also share aggregated, anonymized findings as part of broader international industry research. This is intended to support constructive dialogue, help inform policy in a way that fosters innovation, and ensure that AI development is not unnecessarily hindered, but instead encouraged through informed and balanced approaches.
All responses will be kept strictly confidential and stored securely, with results reported only in aggregate form so that no individual participant or organization can be identified.
Instructions:
- The questionnaire contains mandatory questions that all member organizations should answer (estimated ~30-40 minutes).
- We use a mix of question formats:
- Yes/No (with opportunity to elaborate),
- Multiple-Choice,
- Short Open-Ended prompts (Instructions for responding are provided for each question),
- Rating Scales, a scale of 1 to 5 (where 1 = lowest or “not at all” and 5 = highest or “fully/completely”).
- Please answer each question as completely as possible. For Yes/No questions, if you answer “Yes,” kindly provide a brief description or example. If “No,” you may note any plans or reasons. For open-ended questions, a few sentences are sufficient. For “Not Relevant” please provide a short answer “Why?”.
- THIS IS NOT A TEST – there are no “right” answers. The goal is to encourage self-awareness. Reflect on how your organization handles key aspects of AI ethics. For example, some questions prompt you to “think about a recent AI project” – use such prompts to guide your reflection. Be frank about challenges as well as strengths.
- If you need to pause or gather input from your team, feel free to use the “Save & Continue” feature. This will send a resume link to your email, allowing you to return to the form at any time within 30 days from the date you saved your draft.
Framework Alignment: The questionnaire covers key domains of ethical AI identified by major AI ethics frameworks and guidelines (e.g. OECD, EU AI Act, UNESCO, ISO, IBM, MICROSOFT, APPLE, etc.) – such as human oversight, fairness, transparency, privacy, security, and accountability. This alignment ensures a holistic view of your AI ethics practices in line with international best practices.
